Please download and complete the report, then follow the instructions below to submit the form through CIMS2:

  1. Open the MHA plan
  2. Click Submit/Track
  3. Select the Submit Outcome and Expenditures Report action
  4. Click the Choose File button
  5. Navigate to and select the file
  6. Click the Save button
